Dental Patient Information – Sebastian, FL
Making Dental Visits Easy and Seamless
Appointments with your dentist shouldn’t be overly complicated. Our team at Sebastian Dental wants to take the stress out of every step, including the logistical and financial parts. Below you’ll find information that you will need to know before you come in for your appointment. Please take a few minutes to review this information carefully so that you can be better prepared to have a successful, positive dental experience with us.
Your First Visit
Before you arrive, we ask that you fill out our new patient forms and bring them with you. When we’re ready for you, we’ll call you back and take any necessary X-rays or chart information. Both your hygienist and your dentist will take a few minutes to ask about you personally as well as your dental history and smile goals. Then, they will thoroughly examine your mouth and review the X-rays, explaining their findings and your treatment options to you. Then, our treatment coordinator can go over your insurance benefits and payment options.
If you have dental insurance, you could save a lot of money by partnering with our team at Sebastian Dental. We can help you take full advantage of your coverage and make sure your smile stays healthy for years to come. If you need help understanding your benefits, just ask our knowledgeable team.
We are in-network with the following plans:
- Florida Blue
- United Healthcare
- Delta Dental
- Blue Cross Blue Shield
At Sebastian Dental, we realize that paying the entire cost of treatment all at once may not be feasible or comfortable for many patients. That’s why we accept payment through CareCredit and Aetna Access Discount Plan. CareCredit breaks up the cost into smaller monthly payments that are easier for your wallet to manage. Plus, if you qualify, you may get low or even no interest. Want to learn more about these financing options? Give us a call!
Before we can start your appointment, we need to collect some information from you. Giving us your dental and medical history, insurance information, and more can help us make your appointment run more smoothly. You have the option to complete this paperwork in our waiting room before your visit, but we ask that you fill in the forms before you arrive. That way, you will be ready to go without any delays.
